I’m Tyler Hand, NP-PHC, the founder and clinical director of Xpressions Medical.
I am a Nurse Practitioner in Primary Health Care, and I’ve dedicated my career to building a model of care that puts patients first. Over the years,
I’ve seen how gaps in Ontario’s healthcare system — long waitlists, limited access to family doctors, and short, rushed appointments — leave patients feeling unseen and underserved.​

We envision a future where every patient in Ontario has timely access to primary care led by nurse practitioners, where prevention and wellness are at the center of health, and where women’s health, skin health, and family medicine are integrated seamlessly.
